Instructions
The work permit request process is a multi-step workflow. Learn and follow these instructions if order to do it correctly and avoid delays with the request approval.
Step 1- Download the Risk Assesment Template bellow. Complete this form in all of its parts. Once completed proceed to step 2.
Step 2 – Use the Permit Request Form bellow to request a work permit. Complete the require fields and when prompted attach the completed Risk Assessment Form in Step 1. Once completed submit and look out for email confirmation.
Step 3- Monitor the status of your requests under My Request tab and respond to any other document requests from the EHS team. You may receive automated emails from the system of from EHS approvers with instructions on how to do so.
Risk Assessment Template
Download the risk assessment document template. Complete this form before requesting the permit. The completed risk assessment MUST be attached to the permit request form.
